Activation Instructions:Please complete one or more of the Activation Forms located on the front or inside of your certificate and mail it by certified mail to the address listed on the Activation Form. Include with each Activation Form your deposit of $99 in the form of a cashier check made payable to ETTSI for each Activation Form you submit. The issue date on the Activation Form must be completely filled out by the participating sponsor to validate this certificate. Your Activation Form must reach the validation center within 14 days of your Issue Date. To be eligible for this offer you must be at least 21 years of age, and have a valid major credit card.
